SCJ: Participant Information Form now Provides for Participants to Include Phonetic Pronunciation of their Names

The Superior Court of Justice’s Participant Information Form has been updated. It now invites hearing participants to provide, if they wish, a phonetic pronunciation of their name.

The Form also now replaces counsel slips. It continues to be required in hearings using CaseLines, and can be used in other in-person, hybrid or virtual hearings.

The updated form is available on the Court’s website under Other Documents on the COVID-19 Notices and Orders page. It is also available in the Supplementary Notice to the Profession and Litigants in Civil and Family Matters Regarding the Caselines Pilot, E-Filing, and Fee Payment (paragraph 11 of part 1(C)).
